Scunthorpe produced a miraculous escape to pip Derby in a League Cup first-round penalty shoot-out, after normal and extra-time ended 5-5.
Derby held a two-goal advantage early in the first half at Pride Park, after headers from Richard Keough and Jake Buxton.
The lead was extended to three goals when Theo Robinson converted a strike from the six yard box in the 40th minute.

But two goals deep into injury time to Connor Jennings and Grant saw the visitors into extra-time, which ended scoreless, before they denied the hosts 7-6 in the shoot-out.
Birmingham routed Barnet 5-1 after scoring four goals in a dominant second-half performance.
Barnet took the lead when Joe Nurse finished a header in the 31st minute.
Marlon King responded for Birmingham equalising from the penalty spot, to leave it all-square at the interval.
A powerful strike from Steven Caldwell in the 49th gave Birmingham the lead soon after the break.

Burnley beat Port Vale 3-1 away from home, Ipswich Town accounted for Bristol Rovers 3-1 while Stevenage got past AFC Wimbledon by the same scoreline.
Yeovil Town were comfortable 3-0 victors over Colchester United, while Gillingham progressed past Bristol City 2-1 away from home and Coventry City edged Dagenham & Redbridge 1-0 away also.
Plymouth Argyle dismissed Portsmouth 3-0, Peterborough thrashed Southend United 4-0, Swindon Town impressed in a 3-0 win over Brighton & Hove Albion, Oxford United edged AFC Bournemouth on penalties and Leyton Orient also won a shootout against Charlton Athletic.
